Comments

This information is collected and stored securely using Jotform. PAR-Q data is used solely to assess suitability for training, ensure client safety, and tailor coaching services appropriately.

Health data is classified as special category personal data under UK GDPR and is processed only with your explicit consent. PAR-Q data is retained only for as long as necessary to provide coaching services or to meet legal and insurance requirements.

Embedded content from other websites
